For a patient with a true allergy to codeine, the risk of cross-reactivity with another opioid can be reduced if an analgesic from a different chemical class is used. Chemically, there are three classes of opioid analgesics: phenanthrenes, phenylpiperidine, and phenylheptanones. Morphine and its related opioids, including codeine, are in the phenanthrene class. The mixed agonist-antagonist agents (buprenorphine, nalbuphine, butorphanol, pentazocine, and dezocine) are also included in the phenanthrenes. Use of any of these agents in a patient with a true allergy to codeine may result in an allergic reaction. Meperidine and its related congeners are phenylpiperidines, while methadone and propoxyphene are phenylheptanones. Selecting agents from a different chemical class should minimize the risk of cross-sensitivity. However, it is important to note that use of an opioid from a different chemical class does not eliminate the risk of an allergic reaction. For patients with severe or life-threatening reactions, nonnarcotic analgesics, such as nonsteroidal anti-inflammatory agents or acetaminophen, should be considered. References.

Most people who think they're allergic to codeine aren't actually allergic. An allergic response involves your immune system; antibodies called immunoglobulin E are released and they trigger your mast cells and the production of histamine. People with true allergies to codeine, which is an opioid, can suffer symptoms like hives, rash, low blood pressure, difficulty breathing, and swelling beneath the skin (angioedema). However, most people with codeine "allergies" really suffer from a pseudoallergy that doesn't involve the immune system but can involve histamine anyway. Symptoms of codeine pseudoallergies are flushing, itching, sneezing, hives, sweating, low blood pressure and worsening of asthma. The difference between the allergy and a pseudoallergy is that an allergic response can be caused by even a minute amount of codeine, whereas a pseudoallergic response is typically dose-related.

In addition, codeine can carry side effects even for people with no specific sensitivity. These include drowsiness, nausea, constipation, itching and sometimes even difficulty breathing.

I can say that with over 99% confidence, as a real allergy to codeine is about as common as a customer that is aware they have insurance and.

I know this is a very old post, but I couldn't help commenting on this. Yes, pharmacists and doctors have a lot of education and training. Yes, they have far more understanding of these matters than the average individual. Yes, they deserve respect for the help they provide. But, no, that does NOT give them the right to act like a pompous jackass who thinks they know more than every individual who comes to them. I learned about a few of my pharmaceutical peculiarities by personal experimentation like a reckless teenager.

... migraine headaches. taking 10mg every 4hrs and 40mg 8am and 8pin still is not maintaining the pain. It is getting so bad that it i had to go to.
