Abstract. Removal of the alcoholic hydroxyl in members of the morphine and codeine series and its substitution by hydrogen increases toxicity and convulsant.

Removal of the alcoholic hydroxyl in members of the morphine and codeine series and its substitution by hydrogen increases toxicity and convulsant action, markedly enhances analgesic and depressant effects, decreases emetic effect and usually increases the effect on the intestine (rabbit).

The results of this chemical change are like in kind to those of muzzling of the alcoholic hydroxyl by an acetyl or methyl group, the principal difference being that the muzzling does not increase analgesic and depressant effects to as great a degree.

The most interesting and most powerful of the desoxy-compounds described is dihydrodesoxymorphine-D ("Desomorphine"). This substance is ten times as analgesic (cats) and fifteen times as depressant (rats) as morphine itself, but is only about three times as toxic (mice). It has exhibited no emetic effect and very little convulsant action. Its action is very rapid in onset but also brief in duration. Observations upon its tolerance and addiction properties will be reported shortly. What's this? What's this?

3 Answers - Posted in: demerol, osteoarthritis, fibromyalgia, codeine - Answer: There is always tramadol, and the new med for more severe.

Sustan, I suggest you give Nucynta a try, even though the tramadol didn't do anything for you. Nucynta is in the same drug family as tramadol, but more potent. It's a new drug, though, so just make sure that your insurance covers the cost before going through the trouble of getting an RX for it. Or you can try to get the cost covered by the manufacturer. Either way, it's definiy worth a try for you. 2 Apr 2013.

Pharmacokinetic drug-drug interactions with codeine, dihydrocodeine, hydrocodone, oxycodone, and buprenorphine are reviewed in this column. These compounds have a very similar chemical structure to morphine. Unlike morphine, which is metabolized chiefly through conjugation reactions with uridine diphosphate glucuronosyl transferase (UGT) enzymes, these five drugs are metabolized both through oxidative reactions by the cytochrome P450 (CYP450) enzyme and conjugation by UGT enzymes. There is controversy as to whether codeine, dihydrocodeine, and hydrocodone are actually prodrugs requiring activation by the CYP450 2D6 enzyme or UGT enzymes. Oxycodone and buprenorphine, however, are clearly not prodrugs and are metabolized by the CYP450 2D6 and 3A4 enzymes, respectively. Knowledge of this metabolism assists in the understanding for the potential of drug-drug interactions with these drugs. This understanding is important so that clinicians can choose the proper dosages for analgesia and anticipate potential drug-drug interactions.
